Pathway of Hope
What we offer
Pathway of Hope (PoH) provides individualized client support to help individuals and families working to overcome poverty or crisis and pursue their life goals. This is a strength based case management program where a dedicated caseworker walks alongside participants as they identify needs, set goals, and access resources.
Participants can work on areas such as:
• Setting personal and family goals
• Accessing community resources
• Improving stability and well being
• Building a supportive network for long term change
This program is offered on a first come, first served basis and is open to individuals and families ready to take steps toward positive change.
